Back and neck pain: heat or cold, and when physiotherapy helps

A twinge in the lower back. A neck that won't turn. The first question is usually the simplest: heat or cold? And the second is the expensive one: do I need physiotherapy? Here's what the evidence actually says — including the part about your pillow.

The short answer

For most acute back and neck strains, heat and cold both offer mild, similar short-term relief — the research finds no clear winner, so choose whichever feels better. For lower back pain specifically, continuous heat wrap has the better evidence (moderate short-term pain reduction, even better when combined with exercise); cold's evidence is surprisingly thin. Physiotherapy helps most when it's active — exercises and movement — rather than passive 'treatments' you receive. And since you spend a third of your life in bed, the position and pillow you sleep in are not a side note: a neck-friendly setup and a mattress that supports your spine can be the difference between waking stiff or waking fine. Persistent pain, pain down the leg with numbness, or pain after an injury needs a doctor, not a heating pad.

Back and neck pain is one of the most common health complaints there is — most people experience low back pain at least once. And yet the first-line advice you get online swings between heat, cold, stretching, rest, and 'see a physiotherapist', often without much evidence. Let's fix that.

Heat or cold? The honest answer

For an acute strain — a neck you slept on wrong, a back that seized up — the research comparing heat and cold finds them roughly equal: both produce a mild improvement in pain, and no trial shows a clear winner. A randomized trial of heat vs cold packs for acute neck and back strain found similar, mild relief in both groups (and noted the real painkiller in the room was probably the ibuprofen everyone received). The practical takeaway: heat and cold are both reasonable, both mild, and both safe — choose the one that feels better and is available.

That said, the evidence is not perfectly symmetric. For lower back pain, the better data supports heat: a Cochrane review found continuous heat wrap therapy gives a moderate short-term reduction in pain and disability, and that adding exercise to heat works even better. Cold for low back pain? The evidence is thin — surprisingly thin for something so commonly recommended.

What causes lower back pain

Most acute low back pain has no scary cause found — it's the product of muscle strain, poor loading, prolonged sitting, and sometimes just how the spine is wired. 'Back pain lower spine causes' is a big search; the honest version is that most cases are 'non-specific' — meaning the pain is real, but no single dangerous cause is identified, and it usually settles within days to weeks. The patterns that matter and deserve medical attention: pain radiating down a leg with numbness or weakness (possible nerve involvement), pain after a fall or injury, pain with fever, or back pain that keeps worsening or comes with bladder or bowel changes.

Does physiotherapy help?

Yes — but the useful kind is active. Physiotherapy that gets you moving, exercising, and building strength has good evidence for low back pain: exercises improve function and reduce pain, and adding exercise to heat wrap therapy outperforms heat alone. What has weaker evidence is the passive version — where you lie on a table and receive treatment without a plan for your own movement. The rule of thumb: a good physiotherapist gives you homework. If you're seeing one and you're not being taught exercises and movement strategies, that's a red flag about the treatment, not about you.

Sleep and your back: the part everyone skips

You spend roughly a third of your life in bed, and your spine doesn't clock out. A neck that hurts every morning, or a back that's stiff on waking, is often a sleeping-position and pillow problem as much as a medical one. A few things that genuinely help:

  • Pillow height: your neck should stay roughly in line with your spine. Side sleepers need a firmer, higher pillow; back sleepers a thinner one. A pillow that props your head up at an angle is a recipe for morning neck pain.
  • Mattress support: the goal is neutral spine alignment — not rock hard, not sagging. If you sink in the middle or roll into a hammock, your mattress is working against you.
  • Position: for low back pain, sleeping with a pillow between the knees (side) or under the knees (back) takes pressure off the lower back.

If your pain reliably shows up in the morning and fades during the day, your bed is a bigger suspect than your spine.

Morning stiffness that fades by lunch isn't usually a spine disease — it's a conversation between your spine and your pillow. Fix the bed before you fear the back.

What actually helps, in order

  • Keep moving gently. Bed rest for days is out of date — gentle movement and normal activity recover faster than prolonged rest.
  • Heat or cold, your choice. Mild relief either way; heat has slightly better evidence for the lower back.
  • Exercise. The strongest long-term lever for low back pain — physiotherapy-led or consistent movement you can sustain.
  • Check your sleep setup. Pillow, mattress, position — the third of your life you can control.
  • See a doctor for red flags: radiating pain with numbness, injury, fever, or worsening pain.
